Snowflake schema explorer fix for PAT auth

Feature Request

Fully-qualified INFORMATION_SCHEMA queries for Snowflake under Programmatic Access Token authentication. Resolves the 391918 'must specify database' error for user-created Snowflake databases. Both fetchSchema and loadColumns paths now use fully-qualified queries.
